5th Grade Mental Health Survey

Our school is interested in the behavioral and emotional health of our students. This year, Sun Valley School will be participating in a brief universal screening survey for measuring behavioral and emotional strengths and weaknesses in adolescents. All 5th grade students who agree to participate will complete a short survey during part of one class period during regularly scheduled school activities. This survey has been approved by Sun Valley, in collaboration with researchers from the University of California, Santa Barbara (UCSB).


A detailed information letter was mailed to parents/guardians on October 4, 2018. If you do not want your child to take the survey, you should return this letter with your signature and child's information.
